Principal’s Curricula Authorization Form

The New Mexico GRADS System supplies training and a free implementation kit which contains curricula, videos and supplies valued at over $700, to trained, approved educators for the purpose of implementing Pregnancy Prevention / sexuality education programs in their districts.  

 

Teachers desiring to receive a kit must attend a “mandatory training” on the use of the materials and must obtain their principal’s authorization signature assuring his/her agreement that the course will be offered within one semester of the teacher’s training and that it will be taught in compliance with the local district’s policy for Pregnancy Prevention / sexuality education.   Please document below, your school district’s policy pertaining to supplying students with contraceptive information and the curricula desired for your implementation kit.

 

Our District Policy for “SEX EDUCATION CURRICULUM” is:

 

        Abstinence Education

 

        Abstinence Plus - Contraception Promotion

 

        Abstinence – Our district does not desire contraceptive information taught in the classroom, BUT please supply this information for our school nurse to use as the district policy allows.

 

Implementation Kit Curricula Requested

 

        High School curriculum, Reach for the Stars (includes information on Sexually Transmitted Diseases)

 

        Junior or Middle School curriculum, Game Plan (includes information on STD’s)

 

        OPTIONAL – We are also interested in Upper Elementary curricula, as we are planning to implement district wide prevention programs

 

        OPTIONAL – Teaching Facts About Birth Control Methods: Preparing for Marriage and           

                             Parenthood

 

(This curriculum addresses contraceptive use but does not include the demonstration of condom use.

It reinforces abstinence as the expected standard for teens.)

           

I hereby, authorize and approve of the above and agree that prevention courses will begin within one semester of the teacher’s training. These courses will be used within the guidelines set forth in our district policy for Pregnancy Prevention and Sexuality Education.  I also assure that the teacher will meet all documentation requirements of the GRADS System for the purpose of program evaluation and impact.

As indicated above, GRADS requires the principal’s authorization BEFORE providing a teacher with the implementation kit, which will be distributed to the teacher at the training.  Please assure she/he brings this signed authorization form to the training OR, either fax it to Dawn Orr, GRADS Program Development Coordinator, at 505.838.3720, or mail it to her at: NM GRADS, Attention: Dawn Orr, P.O. The mission of the GRADS Pregnancy Prevention component is to promote healthy relationships and

 reduce Teen Pregnancy, and other risk-taking behaviors among all youth served.

 

(Pregnancy Prevention Programs are provided by the New Mexico GRADS System through funding from the New Mexico Public Education Department.)
